• COVID pre-appointment information & screen

  • We are open for appointments in the clinic and we look forward to seeing you, however, we have  procedures to follow and it’s important that you read the following information:

    Appointment Instructions

    • Please only come into the building ONE MINUTE prior to your appointment, unless you have paperwork to fill out. This is generally only patients presenting for initial consultations that haven’t filled out our web form
    • Come alone: Do not bring anyone else into the clinic with you unless a support person is needed
    • You need to wear a mask and have passed the pre-appointment screen (below)
    • Scan the QR code
    • There will be hand sanitizer available at the door, please use this before entering opening the door
    • EFTPOS or internet banking is the preferred payment method if able
    • We also recommend using the hand sanitizer upon leaving the premises

    Vaccination Pass:          

    You are not required to show this

    Signing in:                 

    You are required to sign in via scanning the QR code or paper form

    Masks:    

    Masks to be worn at all times in the clinic, unless you have a medical reason as to why you cannot – an exemption needs to be shown for this.
    If a mask is not worn and you don’t have an exemption, you will be asked to leave and you will not be seen.

     

    Pre-appointment screen

    Please DO NOT enter the building if you answer yes to any of the below questions:

    • Have you, or anyone you have had close contact with, had a suspected, probable or confirmed case of Covid-19, or are currently awaiting the results of a test?
    • Have you been in contact in the last two weeks with others who have been unwell with respiratory symptoms or fever?
    • Do you have any of these symptoms:
      • New or worsening respiratory symptoms, with or without fever, including one of the following
      • Any cold or flu symptoms
      • sore throat
      • cough
      • shortness of breath
      • runny nose
      • temporary loss of smell
    If you have any of the above, you should call healthline to assess whether you need a test done.
